Heal 360 Clinics and Owner Agree to Pay $20 Million to Resolve False Claims Act Allegations Over COVID-19 Uninsured Program Billing

Heal 360 Urgent Care PLLC, Heal 360 Primary Care PLLC, and their owner, Dr. Mohammed Amer Mohiuddin, have agreed to pay $20 million to resolve False Claims Act allegations that they billed a federal COVID-19 program for office visits that never happened. The settlement resolves claims tied to the Health Resources & Services Administration’s Uninsured Program, which reimbursed providers for testing, treatment, and vaccines given to uninsured patients during the pandemic.

According to the government, Heal 360 operated dozens of walk-up and drive-through COVID-19 testing sites across Texas, including at its Plano clinics. Patients registered, remained in their vehicles, and received nasal swabs—no other medical care was provided. Federal guidance permitted providers to bill a low-level specimen collection code (CPT 99211) for that service. Instead, the government alleged, Heal 360 and Mohiuddin submitted hundreds of thousands of claims using higher-level evaluation and management codes that paid substantially more. To make the claims appear legitimate, they created template-generated medical records listing past medical history and examination findings, prepared by remote scribes located overseas. Mohiuddin was named as the rendering physician on most claims even though he never visited or treated patients at the testing sites.

The matter resolved two qui tam whistleblower lawsuits filed in Texas, and the relators will receive $3.4 million. A related asset forfeiture action against properties Mohiuddin purchased was also resolved, with sale proceeds credited toward the settlement. HHS-OIG assisted the investigation. The claims are allegations only, and no liability has been determined.
